OSU Earth Week - Organization Registration Form

This form will allow you to register your organization for participation during OSU's Earth Week celebration.

Please fill out all of the requested information below. When you submit this form you will receive an email confirmation.

An organization name must be entered.
Please limit the organization name to 75 characters in length.
Organization Description:
An organization description must be entered.
Please limit the organization description to 250 characters in length.
Website Address:
Please limit the website address to 100 characters in length.

Please limit the second address line to 50 characters in length.
